You have the option to buy additional vision coverage to receive a second pair of glasses or additional contact lens allowance during the same calendar year.

You will not receive an ID card for vision insurance. The vision provider's office will locate your coverage based on your SSN and date of birth. 

Informational ID Card from VSP: You can download an informational ID card from the VSP site by following the steps below. Please note, this card will not have your personal information and instead it will have Marvell's VSP plan details. You will still need to provide the last four digits of your SSN to your vision provider's office. 

1. Log into your vsp.com account. If you have not created one, please register.
2. At the top right-hand corner click on “XXXX’s Member Details”
3. Once your member details open, click the “My Member ID Card” link

You have coverage up to $110 for frames purchased through Costco. Please note that Costco is considered an out-of-network provider for any other vision services, so a vision exam at Costco would be charged at the out-of-network rate.

For any questions or concerns, first contact the location where you received services to resolve the situation. If they are not able to resolve the situation, please call VSP customer service 800-877-7195 for further assistance.

Your coverage goes further when you visit an in-network provider, however, if you visit an out-of-network provider, Marvell's plan as some coverage. You can learn more here.

Process For Submitting an Out-of-Nework Claim: If you or a dependent received vision services with an out-of-network provider, you can submit a claim by logging into your VSP portal at www.vsp.com and complete the online claim form. If you prefer to submit a paper form, you can complete this claim form and return it to the provided adddress.

Tips to speed claims processing: Missing or incomplete information will slow down claims processing. Be reimbursement ready by making sure the following are done:

• Copy of itemized receipt or service statement that contain the following:
  • Doctor's name or office name
  • Name of patient
  • Date of service
  • Each service received and the amount paid
• You typically have 12 months from the date of service to submit for reimbursement.
• Make sure all required fields h ave a value and dates are in the following format: Month/Day/Four-Digit Year

Costco: You have coverage up to $110 for frames and up to $200 for contacts purchased through Costco. Many Costco providers are considered an out-of-network for any other vision services, so a vision exam at Costco would be charged at the out-of-network rate. Ask your Costco provider if they participate in VSP network. If so, your eye exam is covered after a $10 copay. If not, submit the out-of-network claim for reimbursement from VSP after exam copay. Please call VSP (800-877-7195) to confirm if the Costco Optometrist is in network.

Walmart, Target and Sam's: Walmart and Sam's are considered out-of-network provider for all vision services. If you choose to visit a provider through these businesses, you can submit an out of network claim form for reimbursement. FOr these out-of-network services you may be eligible to receive up to $50 for an exam, $105 for contacts, and $70 for frames reimbursement.
