Process For Submitting an Out-of-Nework Claim: If you or a dependent received vision services with an out-of-network provider, you can submit a claim by logging into your VSP portal at www.vsp.com and complete the online claim form. If you prefer to submit a paper form, you can complete this claim form and return it to the provided adddress.
Tips to speed claims processing: Missing or incomplete information will slow down claims processing. Be reimbursement ready by making sure the following are done:
- Copy of itemized receipt or service statement that contain the following:
- Doctor's name or office name
- Name of patient
- Date of service
- Each service received and the amount paid
- You typically have 12 months from the date of service to submit for reimbursement.
- Make sure all required fields h ave a value and dates are in the following format: Month/Day/Four-Digit Year
Costco: You have coverage up to $110 for frames and up to $200 for contacts purchased through Costco. Many Costco providers are considered an out-of-network for any other vision services, so a vision exam at Costco would be charged at the out-of-network rate. Ask your Costco provider if they participate in VSP network. If so, your eye exam is covered after a $10 copay. If not, submit the out-of-network claim for reimbursement from VSP after exam copay. Please call VSP (800-877-7195) to confirm if the Costco Optometrist is in network.
Walmart, Target and Sam's: Walmart and Sam's are considered out-of-network provider for all vision services. If you choose to visit a provider through these businesses, you can submit an out of network claim form for reimbursement. FOr these out-of-network services you may be eligible to receive up to $50 for an exam, $105 for contacts, and $70 for frames reimbursement.
